Instructions for the MAT Technical Expert Questionnaire
Electronic Mail / Internet Address – Email ID is mandatory. It is anticipated that Email will be utilized to give the MAT experts periodic updates on the MAT roster or specific projects.
Language Proficiency – If you are proficient in a language (other than English), please write the language on the MAT Technical Expert Questionnaire.
Professional Registration: Please list the type of registration (e.g. AIA, PE, LS, PG, RLA, CPA) and the corresponding States and U.S. Territories where your expertise is applicable (use 2-digit abbreviation).
Disaster Duty: Have You Ever Served on a Post-Disaster Assessment Team? – Please write the name of the disaster and FEMA designated disaster numbers (if known) for the events that you have served on a post-disaster assessment team. Note that post-disaster assessment teams can be initiated by both private and public organizations. Please indicate if the post-disaster assessment team was initiated by FEMA. Other Duties: Please describe other disaster duties that may be applicable that are not part of a post-disaster assessment team.
Technical Fields of Expertise – Using the Expertise Codes Listing, write the expertise codes on the appropriate category on the MAT Technical Expert Questionnaire. It is not necessary to have an expertise code for all four categories (Primary, Secondary, Tertiary, and Other).
Geographic Expertise – Please list States and U.S. Territories where your expertise is applicable (use 2-digit abbreviation). Geographic expertise is defined as being familiar with the areas' building codes and requirements, being familiar with the local construction methods and practices, and having worked your technical fields of expertise in these geographic areas. Use “US” to indicate nationwide expertise.
Physical or other limitations – MAT service often involves strenuous physical activity out-of-doors for long hours without readily available support services. If you have any limitations that you feel would be applicable for MAT assignments, please indicate them on the MAT Technical Expert Questionnaire.
